Annual Review Information for Patients

The code on your invitation letter relates to the condition or combination of conditions you have. If you have more than one condition you may not need separate appointments for each one. The receptionist will advise you when you book your appointment/s.

A – Asthma

  • Bring your inhalers with you to your appointment.
  • Take your medication as normal.

C – COPD

  • Bring your inhalers with you to your appointment.
  • Take your medication as normal.

D – Dementia

  • It would be helpful if you have a carer for them to attend this appointment with you.
  • Take your medication as normal.

G – Coeliac

L – Learning disability

  • It would be helpful if you have a carer for them to attend this appointment with you.
  • Take your medication as normal.

M – Mental Health

  • Take your medication as normal.

T – Thyroid function

  • Take your medication as normal.

V – Vascular (This includes Heart Conditions, Diabetes, Stroke, Kidney function monitoring)

  • Take your medication as normal.
  • Bring an early morning sample of urine to your appointment.

Note: We no longer require you to fast for your annual review appointments which we hope will make it easier for you and allow us to offer greater flexibility with appointment times.

It is important that you have a review annually to ensure safe and effective prescribing of your medication and to detect potential problems before complications arise.

Please let us know if you are physically unable to attend the surgery.
