Repeat Prescriptions

These can be requested using any of the following methods:

1. Come in to reception and fill in a request form or drop off the tear-off slip

2. Post the tear-off slip with S.A.E.

Please note: If you are using the tear-off slip please tick or circle the desired item.

3. Fax your request to 01452-330845 (Glevum Surgery only), 01452-387936 (St. Michael's Branch Surgery only) or 01452-525823 ( Wheatway Surgery only)

4. You may order your prescription online (just click the link below). Please click the link for the surgery from which you wish to collect your prescription.

PLEASE NOTE THAT TELEPHONE REQUESTS ARE NOT ACCEPTED.
In all cases, please allow 2 working days for the request to be completed.
